What to do, step by step
In Italy you do not see a doctor the way you do at home: it depends on how serious it is and on the time of day. Pick the right line.
1. Life in danger: 112
Chest pain, not breathing, fainting, bleeding that will not stop, a drooping face or an arm that will not move, a serious accident, near drowning. Call 112 and stay where you are: the ambulance comes, sometimes with the doctor's car or the helicopter.
2. Urgent but not serious, by day: the CAU
High fever, vomiting, a small cut that needs stitches, a sprain, sunburn, a jellyfish or insect sting, earache, a red eye. Walk into a CAU (urgent care centre) without an appointment: doctors and nurses see you.
3. At night, on Saturdays and holidays: the guardia medica
When the CAU is closed, call the out-of-hours doctor of the area (the number is below, town by town). A doctor listens, advises you, gives you an appointment at the surgery or comes to where you are staying.
4. In summer at the seaside: the doctor for tourists
From June to September the seaside towns open surgeries just for holidaymakers. Go during the opening hours, pay for the visit and keep the receipt for your insurance.
5. Small complaints: the pharmacy
The lit green cross is the pharmacy. The pharmacist gives advice for free and sells over-the-counter medicines; some need a prescription. At night there is always a duty pharmacy.
6. The emergency room
For serious problems, day and night. At the entrance a nurse decides who goes first (triage): the order of arrival does not count. With a small problem you may wait many hours: the CAU is better.
Coming from abroad? What you need
Urgent care is never refused to anyone. Only who pays changes.
European Union, Iceland, Norway, Liechtenstein, Switzerland
Bring your European Health Insurance Card (EHIC, in Italian TEAM): in the public service you are treated like an Italian and pay only the ticket, when there is one. If you forgot it, you pay and ask for a refund at home.
United Kingdom
Your GHIC card (or a UK EHIC still in date) works: the same rules as for EU citizens.
All other countries (United States, Canada, Australia, Asia, Africa, Latin America...)
The emergency room treats you anyway, but afterwards you pay the full regional tariff. Travel with medical travel insurance, keep its assistance number at hand and keep every receipt. Some countries have agreements with Italy: ask your health service before you leave.
Italians from other regions
Bring your tessera sanitaria. At the CAU, the guardia medica and the doctor for tourists you pay the non-resident fee; some regions refund it.
San Marino
It is a separate state with its own health service (ISS): the emergency number is 118 and the hospital is in Cailungo. Ask at the desk what you will pay.

The public service
Hospitals, emergency rooms, CAU, guardia medica, doctor for tourists: run by the health authorities (AUSL in Emilia-Romagna, AST in the Marche, ISS in San Marino). They cost little or nothing, but you do not choose the doctor and sometimes you wait.
Private care
Private outpatient centres, clinics, doctors and dentists: you book by phone, wait little and pay in full. Many travel insurances refund them or pay directly: call your assistance line first.
The volunteers
The Italian Red Cross, the Pubbliche Assistenze (ANPAS) and the Misericordie are volunteer associations: they drive the 112 ambulances on behalf of the public service, cover events and run patient transport. They are not clinics: in an emergency always go through 112.

The doctor's car (automedica)
It is the 118 car with a doctor on board: the 112 control room sends it together with the ambulance in serious cases. It has no surgery and you cannot call it directly. In Romagna there is one, for example, at the hospital of Novafeltria and at Rimini Terme; for summer 2026 no seasonal doctor's car was announced in our Apennines.
Helicopter and mountain rescue
On trails, in woods and caves call 112: it calls out the rescue helicopter and the Alpine and cave rescue. Before a hike tell someone your route and keep your phone charged.
The CAU in the hills
Inland the nearest CAU are at Santa Sofia and Bagno di Romagna (9 am-4 pm), Novafeltria (8 am-8 pm), Mercato Saraceno and Vergato (always open). In the Marche the out-of-hours doctor has offices at Macerata Feltria, Mercatino Conca, Sassocorvaro, Lunano, Urbania and Sant'Angelo in Vado.
The extra summer ambulances
In summer the Marche Region adds rescue posts (PoTES): in 2026 at Gabicce Mare with a nurse from 13 June to 13 September, 8 am-8 pm and also at night from Friday to Sunday, and at Fano round the clock in the same period. On the Lidi di Comacchio the Ferrara health authority adds rescue vehicles. They come when you call 112.

The emergency room app
In Romagna the AUSL app "Pronto Soccorso Romagna" shows the waiting times of the emergency rooms and the nearby CAU.
Ask at the reception
Your hotel knows which CAU, out-of-hours doctor or private doctor is handiest and can call for you.
Sun and heat
In summer the most frequent calls are sunburn, heatstroke and stings. Drink, stay in the shade at midday, use sun cream.
The dentist
For toothache by day go to the CAU; a private dentist sees you by appointment, for a fee.
